I fixed this problem by using a second SCSI disk which was fdisked and
formatted on another system, and installing RedHat 4.2 on it (skipping
the fdisk and formatting steps). Using the second disk as the root, I
then ran fdisk and mke2fs from the installed system on the first SCSI
disk. Then I re-installed RedHat 4.2, but this time I again skipped
the fdisk and formatting steps. This worked fine and I now have a
working system.
The only problem is that the network driver isn't working well with
the network card. This is a card that comes on-board with the
pc164lx. I think it is a matter of jumper settings (since I am not
able to set the host IP address via the AlphaBIOS permanently), but I
will look into it tomorrow.
So my understanding is that there's something wrong in the RedHat
fdisk or mke2fs found in the root system disk provided by RedHat.
