Yes, I've seen it and it's a corruption (or mis-setup of /etc/pam.conf).
I saw it on a 4.0.4 Intel box and cured it by overwriting the installed
pam.conf with the one from my EB164. I then had trouble with the lock
file (read adduser) which was cured by deleting it.
