You are right -- it is missing from the ftp distribution. How odd. Erik?
You should be able to use any even resonably recent version of MILO.
All that's required is that it know how to boot ELF kernels and that
it know how to talk to your hardware. The kernel and milo numbers
only have to match to build milo -- it says nothing about what kernels
milo can boot.
But fwiw, ftp://gatekeeper.dec.com/pub/DEC/Linux-Alpha/Miniloader/
is the distribution point for MILO.

I've never gotten these, but they certainly don't look good.
Have you built you own kernel, or are you using Red Hat's? If
your own, would you look up some the PC and R[AP] numbers in
the System.map and report the rest of the oops?
