On my box the "intimidating messages" were indicative of a total failure
to re-read the partition table from the disk. The subsequent format
steps failed completely! After a few false starts, I made it past this
point ONLY by re-booting and working through the install routine until I
arrived back at this point (but without partitioning again,
naturally..).
RedHat even notes this glitch in their documentation for release 4.0
(which I didn't have at the time I initially ran into this), and
suggests the same workaround.

However, let me say that I _do_ appreciate Stephen's work in putting
this together. I wouldn't have had a chance at getting through the
install of either 3.03 or 4.0 without it as a reference!
