This error message is indicative of improper termination. I've seen it
with literally all ev4 alpha stations with an integrated NCR
controller. Check the J13 jumper on the motherboard, it's right behind
the floppy interface. Make sure its settings match this info. To
address the fact that first person who responded says it happens off and
on. You either have another unknown scsi problem and your jumper is set
correctly or you are just getting lucky from time to time. SCSI can be
funny like that :)

SCSI Terminator Jumper (J13)
J13 is a 2-position (1 x 2) header. When installed, the SCSI terminator
jumper enables SCSI–2 termination. SCSI termination is not a
software-configurable
feature. Always enable the terminator when this MLB is at the end of the
SCSI
bus. Devices at the end of the SCSI bus must have the terminator
installed or enabled.
